Blue Advantage (HMO) and Blue Advantage (PPO)
dental benefit is administered through a company called United Concordia.
Your Blue Advantage ID card includes the dental information. When you make a dental appointment, please be sure to tell the office staff you have Blue Advantage dental benefits through United Concordia.
Also, be sure to confirm that the dental office is still participating with United Concordia and taking Blue Advantage members. Show your Blue Advantage ID card to the dental office staff when you arrive at each visit.
To take full advantage of your dental benefits, you must use a dentist in the network. The plan covers both preventive and comprehensive services (see list below).
The amounts you pay for these services will not count toward your maximum out-of-pocket amount.
These benefits do not apply to Medicare-covered dental services, such as dental care required to treat illness or injury, which may require prior authorization.

Note: One every year means one every 12 months, not calendar year. (For example, one oral exam performed in June would mean the next oral exam could be performed the following June.)
