Vision Benefits

We are pleased to offer routine vision benefits to all of our members. This benefit helps with the costs associated with your annual eye examination as well as costs associated with lenses and eyewear.

What services are covered under my vision benefit?

Your vision benefit covers one routine eye exam and one pair of eyeglass lenses or contact lenses each calendar year, as long as they are purchased from an in-network routine vision care provider. It also covers $130 toward the cost of eyeglass frames or contact lenses.

What is not covered under my vision benefit?

  • Contact lens fitting
  • The cost of frames above $130
  • Extras such as progressive or featherweight lenses and scratch-resistant coatings

What do I have to pay under my vision benefit?

  • $0 copayment for one routine eye exam per year
  • For eyeglass lenses and frames or contact lenses, limited to one pair every year, there is a $0 copayment. For frames OR contact lenses (conventional or disposable), plans cover up to $130.
  • Any extras (e.g., progressive lenses, featherweight lenses, or scratch-resistant coatings)

Do I have to go to specific vision providers?

Yes, to use your vision benefits you must use a “Routine Vision Care Provider” for routine eye exams (no eye-related problems) or to obtain eyeglasses or contact lenses. Important information regarding Eye Refractions

Eye Refractions are the tests used to determine the prescription for eyeglasses or contact lenses. Traditional Medicare does not cover eye refractions. However, Davis Vision does when provided by a Routine Vision Care Provider. Eye refractions are not covered by Davis Vision when provided by a specialist as part of medical treatment for an eye condition. If you receive medical treatment for an eye condition and the specialist wants you to have eye refraction, you may have to pay one copay to your specialist for medical treatment and another copay to the Routine Vision Care Provider for another eye exam and refraction.
